Certified GeoExchange Designer (CGD) NOTE 2  
       
  ** Business memberships are defined as follows:  
Certified Vertical Closed Loop Driller (CVCLD) NOTE 3  
                 
    General Business membership  
Certified Geothermal Inspector (CGI) $100 per year  
     -  Any business related to the GSHP industry that does not fall under the corporate membership levels (below)    
       
Certified Residential Designer (CRD) $100 per year  
     -  Any school, educational or non-profit entity    
     -  Membership includes two (2) total votes, one (1) designate vote in any sector that is NOT utility or manufacturer and one (1) alternate vote in any sector.                  
        NOTES:              
        Requires AI renewal. Only available to members who have attended train-the-trainer.
     -  Renewal is for a term of 1 year.   CGD certification is with AEE. Check this box to obtain membrership discount.
    Small Corporate Membership   CVCLD certification is with NGWA. Check this box to obtain membrership discount.
     -  Manufacturers of 100 or less employees                  
     -  Utility providers with 10,000 or less meters                  
     -  Membership includes three (3) total votes: One (1) designated vote cast under either the manufacturer or utility sector and two (2) alternate votes cast in any sectors desired.

    Large Corporate Membership                
     -  Manufacturers of 100 or more employees    
     -  Utility providers with 10,000 or more meters    
     -  Generation and Transmission (G&T) providers of any size    
     -  Membership includes four (4) total votes: One (1) designate vote cast under either the manufacturer or utility sector and three (3) alternate votes cast in any sectors desired.
